As an internal medicine hospitalist, I founded Easy Transitional Care Clinic after witnessing too many patients struggle during the critical transition from hospital to home. Patients and families repeatedly expressed the need for more support after discharge—and that need inspired this clinic. Our virtual telemedicine clinic was created to bridge that gap, providing timely follow-up, medication review, education, and personalized support to patients locally and nationwide. We help prevent avoidable complications and hospital readmissions while making recovery less confusing and more manageable. At Easy Transitional Care Clinic, we believe great care doesn’t end at discharge—it continues until every patient feels supported, confident, and connected.

A transition of care telehealth clinic provides short-term virtual care after you leave a healthcare facility. We help you understand your care plan, manage medications, coordinate referrals, and recover safely at home—while working alongside, not replacing, your primary care provider.

We aim to contact you within 24 hours after discharge, and no later than 48 hours.

We are a short-term service that supports you after discharge. You should continue care with your primary care provider, and we help bridge the gap if you don’t have one or can’t be seen quickly—so you’re never left without support during recovery.

Visits are done by video or phone. We review your records ahead of time and send reminders, consent forms, and your visit link before your appointment so everything is simple and ready for you.

If your providers use a connected medical record system, they can see your records and our visit notes. We work within these systems to keep your care coordinated and secure.

Most insurance plans cover this service, though some may require a copay. Self-pay options are also available for patients without insurance.
