What is a TOC telehealth clinic?
A transition of care telehealth clinic provides short-term virtual care after you leave a healthcare facility. We help you understand your care plan, manage medications, coordinate referrals, and recover safely at home—while working alongside, not replacing, your primary care provider.
How soon will they contact me?
We aim to contact you within 24 hours after discharge, and no later than 48 hours.
Do I still need to see my primary care provider ?
We are a short-term service that supports you after discharge. You should continue care with your primary care provider, and we help bridge the gap if you don’t have one or can’t be seen quickly—so you’re never left without support during recovery.
How are video visits conducted?
Visits are done by video or phone. We review your records ahead of time and send reminders, consent forms, and your visit link before your appointment so everything is simple and ready for you.
What kind of support do we provide?
- Nurse check-in within 48 hours of discharge
- Follow-up visit with a provider within 7–14 days
- Help understanding your medications, discharge instructions, and care plan
- Assistance with appointments, referrals, and prescriptions
- Support for new or ongoing symptoms and concerns
Will my primary care provider or discharging facility be able to access my records from your clinic?
If your providers use a connected medical record system, they can see your records and our visit notes. We work within these systems to keep your care coordinated and secure.
Most insurance plans cover this service, though some may require a copay. Self-pay options are also available for patients without insurance.
